First please tell us about your club.

Each club on Clubforce will be provided with a connected Stripe account which will allow you to receive payouts directly into your bank account.

In order to set this up, we require contact details and proof of identity for an authorised representative.

Please note that it is important that all the information provided in the form (including address and date of birth) matches the details on your identity documents as otherwise our payment processor will reject the details.

If you have already provided these details for your club previously, please indicate this below.

Please provide either a clear scan of the photo page of a passport and front and back of the driving licence for the authorised representative. All scans must be in colour and details of the identity document must be clearly visible (no shadows or reflections).
