Stripe Activation - Clubforce

We are updating the payment processor we use at Clubforce to Stripe. As part of this process we require some extra information to setup and verify the identity of an authorised representative from the club. Requested URL for NextGen platform

Each club on the Clubforce NextGen platform will be hosted at a custom URL such as clubname.clubforce.com. We will automatically set this based on your club name but if you have a specific URL you would like to request please add it here. For example, you may prefer dublinfc.clubforce.com to dublinfootballclub.clubforce.com

Your authorised representative details

Each club on Clubforce NextGen will be provided with a connected Stripe account which will allow you to receive payouts directly into your bank account. In order to set this up, we require contact details and proof of identity for an authorised representative. Please note that it is important that all the information provided in the form (including address and date of birth) matches the details on your identity documents as otherwise our payment processor will reject the details.

Please provide both a clear scan of the photo page of a passport and the front and back of a driving licence for the authorised representative. All scans must be in colour and details of the identity document must be clearly visible (no shadows or reflections).
