Full club address, including Eircode/Postcode
Details of your Membership options/plans and fees, including any possible discounts we may need to consider

Upload a document with all your fees clearly outlined
You will also be asked if there are any additional fields/questions your club plans use or need e.g Volunteering, Code of Conduct, Medical Information etc.
You also have the ability to add in Club specific questions. You can if you like upload supporting documentation at this point, e.g. Code of Conduct.

Copy of club bank details, displaying account name, IBAN and BIC (e.g. a bank statement or club-headed paper)
